Made a Difficult Choice
This achievement/trophy’s description is ambiguous. Hopefully this guide will simplify it for you. First, you need to find the Infested Abyss which spawns behind the waterfall under the mural where you initially meet Walt. It is always in the 1st world seed, but you can quickly run there in any random seed and verify that it spawned. Simply travel to the first area and run to and jump through the waterfall.
Work your way through the 3 Root Nexus doors and talk with Root at the end of the area. Don’t antagonize the NPC. Instead, agree to submit to it and you will receive the Profane Soul Stone amulet.
Return to Ward 13 and change to Campaign mode. Then, fast travel to The Labyrinth’s Inner Sanctum. Make sure you equip the Profane Soul Stone amulet and speak with The Keeper. As you leave, it will notice the amulet as offer to cleanse it. Say that you are going to keep it.
Travel back to Ward 13, change to Adventure mode, and return to Root in the Infested Abyss. Having refused the offer from The Keeper, now agree to hand the amulet back to Root. Doing so will unlock The Burden Hardest to Bear achievement or trophy.
